Nursing Relaunch Application

The UT Tyler School of Nursing (SON) Relaunch Program provides an opportunity for students to reenter the SON undergraduate Pre-Licensure BSN track and continue their nursing education. Students may be eligible to apply to the program if they were dismissed for non-compliance with the SON 4.9 Progression and Retention Policy.

The program is designed to support students in achieving academic success through proactive engagement, accountability, and consistent mentoring. Students who are readmitted through the Relaunch Program must adhere to the guidelines outlined in the Relaunch Program Agreement.

The Relaunch Program is a selective process. Applying for the Relaunch Program does not guarantee readmission to the Pre-Licensure BSN track. Placement is based on seat availability at the Tyler Campus.

A decision on your application cannot be made until final grades are posted at the end of the current semester.

Not all students are eligible to apply for the Relaunch Program. Please review the admission criteria outlined on the Relaunch Program Agreement Form before progressing with this application:

Incomplete applications will not be reviewed or considered for readmission. Remember, to be able to submit your complete application you will be required to upload the following documents:

Relaunch Program Agreement Form, complete and signed.
Relaunch Program Performance Improvement Plan, complete and signed.
Updated Pre-Licensure BSN degree audit signed by your academic advisor.

Indicate the last level of the nursing program in which you successfully completed all courses in that level.

For example, If you successfully completed all courses in level 1 (NURS 3307, 3308, 3309, and 3605) but were unsuccessful in NURS 3615 in level 2, you would select "Level 1" as the last level you completed.
